When to register

Registration is done during our normal distribution hours. (Please check Hours of Operation)

If you are new to this pantry, registration is done at your first visit to the pantry. Please arrive at least 15 minutes prior to close so that registration and shopping can be completed before closing.

Registration Process

A registration card is completed for each client of the food pantry. It contains information about you, your income, and the number of people in your household. The information collected is confidential and is not shared outside of the food pantry staff.

You will need to bring the following in order to complete the registration process.

1. A form of identification for every household member. 

2. Current proof of address For every adult household member. 

3. Monthly Income is self-declared and does not require documentation.

Acceptable forms of identification and address verification are:
Wisconsin immunization registry records, report cards, birth or baptismal certificates, passports, medical insurance, library cards, driver’s licenses, state/military/tribal IDs, or first-class mail postmarked within the past 30 days.

Proxy Form

For registered clients who may have become sick or injured or otherwise not able to get to the pantry, a Proxy Form can be completed that will allow a client to designate another individual to pick up your grocery items for you. The proxy is specific to a period of 1,3,6,9 or 12 months as you choose and will expire after that period. 

Your designated proxy will be asked for proof of identification when picking up for you and will asked to sign your registration card indicating you have received your distribution for that month. If a proxy picks up for you, that is your distribution for that month and you will not be permitted another until the following month.
